Introduction: Maternal mortality ratio for Uganda has remained high despite the different measures to prevent maternal mortality. Uterine rupture remains one of the main direct causes of maternal and perinatal mortality and morbidity in sub-Saharan Africa. In Uganda studies done in different parts of the country show that uterine rupture is associated with high maternal and perinatal mortality (Mukasa et al., 2013). However we do not know what risk factors predispose women to uterine rupture in Mulago hospital. Risk factors for uterine rupture in Mulago have not been studied in the last ten years. This study was to determine the risk factors for uterine rupture in women admitted in Mulago hospital 5C. Methods This was a case control study. Cases were defined as patients diagnosed with ruptured uterus. Controls were patients admitted and delivered normally. Questionnaire and case report forms were used to collect data from women diagnosed and managed for uterine rupture. Controls were any three women who delivered normally after the case. Quantitative data analysis was used. Descriptive statistics were obtained using frequencies and cross tabulations. The results were presented as Odds Ratios and confidence intervals. Factors with a p-value of ≤0.05 at multivariate level were considered as statistically significant. Results We recruited 45 cases and 132 controls. The factors significantly associated with uterine rupture were previous caesarian section scar (OR=28.17 95% CI 8.52 - 93.08, p-value<0.001) and lack of a delivery plan (OR=16.59 95% CI 3.99 - 69.03, p-value<0.001). Lack of antenatal care (4visits) was not associated with uterine rupture (odds ratio 1.30, CI 0.62 - 1.84; p-value 0.10)en_US
