    Abstract
    This report presents an online immunization repository. The objective of the study is to design a web based immunization repository that avails immunization information to stakeholders when they need it. The methodology used to collect data was documentary analysis observations and tools used in system analysis and designs were data flow diagrams and entity relation diagrams. The tools used in implementing the system are MySQL for the data base and PHP for back end. The results and findings were that the most important issue that study has revealed is that most health centres lack coherent, integrated management information system that can efficiently and effectively serve information requirement for decision making about immunization activities in the particular centres.
