    Abstract
    The project was about developing a web-based information system for reporting Adverse Veterinary Drug Reactions for the Ministry of Agriculture. The project involved systems study, analysis and design, implementation and testing and validation . The techniques used to achieve the general objective of the project included observation, interviews and documentation reviews, Enhanced Entity Relationship and dataflow diagrams for analyzing and hence modeling the collected data, PHP, MySQL and Wamp server for implementing the programming logic of the proposed system and finally conducted unit, integration and system testing for testing and validating the system. The problem faced by the existing system is the delay of ADR reporting to the Ministry
