Makerere University Hospital is still at a very low level of Information Technology development. It uses the manual paper based Information System to conduct most of its activities. Imperative to note is that the paper based Information System can not be done away with fully because it is still efficient in dealing with some of the hospital‟s business processes. However, issues arise when the processes and operations at the hospital are slow, data and information is misplaced and numerous errors in capturing of data when using the paper based Information System. The final result is poorly analysed information about patients and diseases which make it hard for the hospital administration to make decisions and keep an accurate follow-up on patients and diseases. This report gives a clear presentation of steps taken to come up with a Disease Registry System for Makerere University Hospital. The system is meant to keep track of a chronic disease (Asthma) while monitoring a specific population (Makerere University Students). This report entails clearly stated goals of what the project must achieve. It also focuses on previous literature about the Disease Registry System. Included in the report are the data collection techniques, system analysis and design techniques, development, implementation and testing of the system. The development and implementation of the system was done using HTML (Hyper Text Mark-up Language) which was used mainly to create the interface. Java Script was also used as the client side scripting language to perform input/form validation and producing other features like the calendar. Java Servelets was used as a server side scripting language and connects to the database to retrieve and store data. Hence this project provides Makerere University Hospital with a clear, simple to use and efficient way of analysing and monitoring a particular disease, so as to give its clientele proper and better services. However this system handles one type of disease and the medical aspect has not been fully represented.
